Request for Summer Work Form   

Due to registrar by July 1, 2021

Complete the Request for Summer Work Form.

(This is also available at the Registrar and at the OCEL.)

This form must be signed by:

1. Pre-major/major advisor,
2. Faculty sponsor, and
3. Director of OCEL (this is the last person to sign the form)

Register for 290 in the Department of your Faculty Sponsor. This form is due to the registrar by July 1, 2021.
